個人資料之告知

兄弟大飯店股份有限公司（以下簡稱本公司）為辦理觀光旅館等相關業務活動之目的（「170觀光行政、觀光旅館業、旅館業、旅行業、觀光遊樂業及民宿經營」、「148網路購物及其他電子商務服務」）而蒐集您下列個人資料類別：姓名、信用卡卡號、性別、連絡方式(包括但不限於電話號碼、E-MAIL)等或其他得以直接或間接識別您個人之資料。
本公司將依據個人資料保護法之規定，於特定目的範圍內蒐集、處理您的個人資料，並在特定目的之存續期間或其他公務機關請求行政協助之目的範圍內，於中華民國領域內合理利用您的個人資料。
對於您所提供之個人資料，可依個人資料保護法第3條規定，向本公司行使之下列之權利請求：
1. 查詢或請求閱覽。
2. 請求製給複製本。
3. 請求補充或更正。
4. 請求停止蒐集、處理及利用。
5. 請求刪除。
若您因行使上述權利而導致對您權益產生減損時，本公司不負相關賠償責任。另依個人資料保護法第14條之規定，本公司得酌收行政作業之必要費用。
若您未提供正確之個人資料，本公司將無法為您提供特定目的之相關服務。
若因業務需要，本公司委託其他機關處理您的個人資料時，本公司將善盡監督之責。
您瞭解此一同意書所載內容符合個人資料保護法之規定，並且同意本公司留存。

Disclosure of Personal Information

Brother Hotel Co., Ltd. (hereinafter referred to as the “Company”) collects the following categories of personal data for the purposes of conducting business activities related to tourism hotels, including but not limited to: “Tourism Administration and Hotel Industry, Travel Industry, Tourism and Entertainment Industry, and Bed and Breakfast Operations” (e.g., code 170) and “Online Shopping and Other E-commerce Services” (e.g., code 148), including but not limited to: name, credit card number, gender, contact information (including but not limited to phone number, email), or other information that can directly or indirectly identify you.
The Company will collect, process, and utilize your personal data within the scope of specific purposes in accordance with the Personal Data Protection Act, and will reasonably use your personal data within the territory of the Republic of China during the duration of the specific purposes or for purposes related to requests for administrative assistance from other government authorities.
You may exercise the following rights with respect to the personal data you provide under Article 3 of the Personal Data Protection Act:
1. Request access to or request a copy of the data.
2. Request the provision of a copy of the data.
3. Request supplementation or correction of the data.
4. Request the cessation of collection, processing, or use of the data.
5. Request deletion of the data.
Should exercising these rights result in a diminution of your interests, the Company shall not be liable for any related compensation. Furthermore, in accordance with Article 14 of the Personal Data Protection Act, the Company may charge a reasonable administrative fee for processing such requests.
If you do not provide accurate personal data, the Company will be unable to provide the relevant services for specific purposes.
In cases where it is necessary for business operations, should the Company entrust another organization to process your personal data, the Company will fulfill its supervisory responsibilities to ensure compliance with the applicable data protection requirements.
You acknowledge that the contents of this consent form are in compliance with the Personal Data Protection Act, and you consent to the Company retaining your personal data.

I have been fully informed of the above disclosure of information provided by your company.
I consent to the collection, processing, and utilization of my personal data by your company, as well as its provision for administrative assistance purposes requested by other government agencies.

ブラザーホテル株式会社（以下「当社」）は、観光ホテル等に関連する業務活動（「170観光行政、観光ホテル業、宿泊業、旅行業、観光レジャー業及び民宿経営」、「148 インターネットショッピング及びその他の電子商取引サービス」）を行う目的で、以下の個人情報を収集いたします：氏名、クレジットカード番号、性別、連絡先（電話番号、Eメールなどを含むがこれに限らない）、その他直接的または間接的に個人を識別可能な情報。
当社は、個人情報保護法に基づき、特定の目的の範囲内で個人情報を収集・処理し、その目的が存続する期間、または他の行政機関からの協力要請に基づき、中華民国地域内で適切に利用いたします。
お客様が提供した個人情報に関して、個人情報保護法第3条に基づき、以下の権利を行使することができます：
1. 情報の確認または閲覧の請求。
2. 情報のコピー提供を請求。
3. 情報の補足または訂正を請求。
4. 情報の収集、処理及び利用の停止を請求。
5. 情報の削除を請求。
上記の権利を行使したことにより、お客様の利益が減少した場合、当社は責任を負いません。また、個人情報保護法第14条に基づき、当社は行政手続きに必要な費用を請求することがあります。
お客様が正確な個人情報を提供しない場合、当社は特定の目的に基づくサービスを提供できない場合があります。
当社が業務上、他の機関に個人情報の処理を委託する場合、当社は適切に監督責任を果たします。
お客様は、この同意書に記載された内容が個人情報保護法に準拠していることを理解し、当社がその情報を保管することに同意します。
